Why this applies to Iptacopan
Iptacopan falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES — the group Cannabidiol (cbd) interacts with.
In vitro research shows that cannabidiol inhibits CYP2C8. However, this interaction has yet to be reported in humans. Until more is known, use with caution. Theoretically, concomitant use of cannabidiol with CYP2C8 substrates might increase the risk for adverse effects from these substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Fabhalta (iptacopan), a medicine you take by mouth to help protect your blood cells in conditions like PNH. Part of how your body clears iptacopan runs through a liver enzyme called CYP2C8. CBD appears to slow that enzyme down, at least in lab dish studies. If it does the same thing in people, iptacopan could build up a bit higher than usual, which in theory could nudge up side effects like headache, belly pain, diarrhea, or nausea.
I want to be honest though: this is theoretical. It's only been seen in test-tube research, not in real patients, so we don't know if it truly matters day to day. Iptacopan isn't a prodrug, so more CBD wouldn't weaken it. Before pairing the two, please run it by your pharmacist or doctor.
