Macitentan, Tadalafil falls under ANTIHYPERTENSIVE DRUGS — the group Quercetin interacts with.

Theoretically, taking quercetin with antihypertensive drugs might increase the risk of hypotension.

Quercetin can modestly decrease blood pressure in people with mild hypertension. Theoretically, it might have additive blood pressure lowering effects when used with antihypertensive drugs.

Opsynvi combines two blood pressure lowering medicines: macitentan (which relaxes and widens blood vessels in the lungs) and tadalafil (the same active ingredient found in Cialis, which also relaxes blood vessels). Both can gently lower your blood pressure on their own. Quercetin has been shown in some smaller studies to modestly bring blood pressure down too.

So the concern here is simple: stacking quercetin on top of Opsynvi could add up and push your blood pressure a little too low. That might leave you feeling lightheaded, dizzy, flushed, or faint, especially when you stand up quickly. This is mostly a theoretical, additive effect, and everyday food amounts of quercetin are unlikely to matter. Before starting a concentrated supplement, please check with your pharmacist or doctor.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Macitentan, Tadalafil also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Opsynvi is a combo pill that pairs macitentan with tadalafil, and both are partly cleared by a liver enzyme called CYP2C9. Quercetin in concentrated supplement form can gently slow that enzyme down. In theory, that could let a bit more of these medicines linger in your body, which may nudge up their usual side effects. For tadalafil that might mean more headache, flushing, stuffy nose, dizziness, or an upset stomach. Because both drugs also lower blood pressure, feeling lightheaded when you stand up is the main thing to watch for.

Keep in mind this is mostly theoretical and based on lab work plus a small study using other drugs, so the real-world effect is uncertain. Everyday food amounts of quercetin are unlikely to matter. Before adding a high-dose supplement, check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Macitentan, Tadalafil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Opsynvi combines two active medicines, macitentan and tadalafil, and both are cleared by the liver enzyme CYP3A4. Quercetin can nudge that enzyme, though the research is mixed. In lab and animal studies it mostly slows CYP3A4, which could let these drugs build up a little. If that happens, you might notice more of their usual effects, like low blood pressure, dizziness, headache, flushing, or a stuffy nose. Interestingly, one human study showed quercetin actually speeding up a similar drug's breakdown, so the direction is not clear-cut.

The good news: this is mostly theoretical, and everyday food amounts of quercetin are very unlikely to matter. High-dose supplements are the real question. Before adding one, please check with your pharmacist or doctor.
