Quercetin and Nabumetone

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Quercetin interacts with whole categories of medication. Nabumetone falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES

1 of 2

Why this applies to Nabumetone

Nabumetone falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the effects and adverse effects of OAT3 substrates.

In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with these two together. Your body uses a transporter called OAT3 to help move certain drugs out through the kidneys. In the lab, quercetin blocks that transporter pretty strongly. Since nabumetone (well, its active form) relies on routes like this to be cleared, slowing it down could let a little more of the drug linger in your system.

If that happened, you might notice more of nabumetone's usual NSAID side effects: stomach upset or irritation, fluid retention, higher blood pressure, or in some cases kidney strain. But I want to be honest with you, this is only based on test-tube research and has never been reported in real people. Food amounts of quercetin almost certainly won't matter. Before starting a high-dose supplement, just run it by your pharmacist or doctor.

ORGANIC ANION TRANSPORTER 1 (OAT1) SUBSTRATES

2 of 2

Nabumetone also falls under ORGANIC ANION TRANSPORTER 1 (OAT1) SUBSTRATES.

Theoretically, concomitant use might increase the effects and adverse effects of OAT1 substrates.

In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT1, with half-maximal inhibitory concentration (IC50) values less than 10 mcM. So far, this interaction has not been reported in humans.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with these two: a protein called OAT1 helps your kidneys move certain drugs out of your body. In lab dish studies, quercetin seems to block OAT1 fairly strongly. If that same thing happened in people, the active form of nabumetone could clear a little slower and build up, which might mean stronger effects and more of the usual NSAID side effects (stomach upset, heartburn, fluid retention, higher blood pressure, or kidney strain).

The important part: this has only been seen in test tubes, never reported in actual patients, so it's theoretical. Food amounts of quercetin almost certainly don't matter. If you're taking concentrated quercetin supplements, just run it by your pharmacist or doctor before combining them.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Nabumetone page

“Five of the 52 supplement and herbal ingredients on file for nabumetone (Relafen) rate major, and those five are where real vigilance belongs. Nabumetone is an NSAID taken for arthritis and joint pain, and NSAIDs already stress the stomach and the kidneys, so the worst pairings hit exactly those weak spots.”

  • Heart and blood pressure health
  • Anti-inflammatory support
  • Immune support
  • Exercise recovery

Quercetin is a plant flavonoid with antioxidant and anti-inflammatory properties found in many common foods and sold as a supplement. While early research is interesting for allergies, blood pressure, and inflammation, strong human evidence is limited for most uses. It is generally well tolerated in food amounts, but talk to your pharmacist or doctor before taking high-dose supplements.
