Quercetin interacts with whole categories of medication. Niacin, Lovastatin falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Niacin, Lovastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Advicor combines niacin with lovastatin, and it's the lovastatin part that matters here. Lovastatin is cleared by an enzyme called CYP3A4, and quercetin may block that enzyme in lab studies. If it does, lovastatin could build up in your body. That can raise the odds of statin side effects like muscle aches, soreness, or weakness, and rarely a more serious muscle problem.
That said, this is mostly theoretical and based on lab and animal work, not solid human studies on this exact combo. One study even hinted quercetin might speed the enzyme up instead. Food amounts of quercetin are unlikely to cause trouble. If you're considering a high-dose supplement, run it by your pharmacist or doctor first.

ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP)
2 of 2Niacin, Lovastatin also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).
In vitro evidence shows that quercetin can inhibit organic anion-transporting peptide (OATP) 1B1-mediated uptake of estrone-3-sulfate and pravastatin. Furthermore, clinical research in healthy males shows that intake of quercetin along with pravastatin increases the AUC of pravastatin by 24%, prolongs its half-life by 14%, and decreases its apparent clearance by 18%, suggesting that quercetin modestly inhibits the uptake of pravastatin in hepatic cells.

Advicor combines two cholesterol medicines: niacin and lovastatin. The piece we care about here is lovastatin, which gets shuttled into your liver partly by transport proteins called OATP. Quercetin can gently slow those transporters down. When that happens, a little more lovastatin can hang around in your bloodstream instead of being cleared, which may nudge up the chance of side effects like muscle aches, soreness, or weakness, and rarely liver irritation.
That said, this is mostly theoretical and based on lab work plus a small study with a related statin (pravastatin), where the bump was modest. Quercetin from everyday foods is unlikely to matter. High-dose supplements are a different story, so before starting one, check with your pharmacist or doctor so they can keep an eye on your muscle symptoms.
