P-GLYCOPROTEIN SUBSTRATES

Ombitasvir, Paritaprevir Ritonavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Quercetin interacts with.
There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist with Technivie (ombitasvir, paritaprevir, ritonavir) and quercetin. Quercetin may block a gut transporter called P-glycoprotein, which acts like a bouncer that pushes some drugs back out of your intestines. If that bouncer slows down, more of certain medicines can get absorbed, so blood levels can creep up. With this hepatitis C combo, higher levels could mean more of the usual side effects, like nausea, tiredness, trouble sleeping, or itching.
Honestly, this is mostly theoretical and built on small, mixed studies, so I don't want to alarm you. Food amounts of quercetin are unlikely to matter. Concentrated supplements are the bigger question. Before adding a quercetin supplement, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Ombitasvir, Paritaprevir Ritonavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist with Technivie (ombitasvir, paritaprevir, and ritonavir). Parts of this combo, especially paritaprevir, are cleared by a liver enzyme called CYP3A4. Quercetin can nudge that enzyme, though the lab and small human studies pull in both directions: some suggest it slows the enzyme (which could let drug levels build up, raising the odds of side effects like nausea, tiredness, itching, or trouble sleeping), while one study hinted it might speed the enzyme up (which could lower levels).
Honestly, this is mostly theory and weak data, not something proven in real patients. Food amounts of quercetin are very unlikely to matter. Before starting a high-dose supplement alongside this hepatitis C treatment, just check with your pharmacist or doctor first.

The combination of ombitasvir, paritaprevir, and ritonavir is usually used in combination with ribavirin (Copegus, Rebetol), but sometimes it is used alone to treat a certain type of chronic (long-term) hepatitis C infection (swelling of the liver caused by a virus). Ombitasvir is a hepatitis C virus (HCV) NS5A inhibitor. It works by stopping the virus that causes hepatitis C from spreading inside the body. Paritaprevir is a protease inhibitor. It works by decreasing the amount of HCV in the body. Ritonavir is a protease inhibitor. It helps to increase the amount of paritaprevir in the body so that the medication will have a greater effect. It is not known if ombitasvir, paritaprevir, or ritonavir prevent the spread of hepatitis C to other people.
