Why this applies to Pacritinib

Pacritinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to help clear Pacritinib (Vonjo), the medication used for myelofibrosis. Quercetin can tinker with that same enzyme. In most lab and small human studies it appears to block CYP3A4, which could let more Pacritinib build up in your blood. That might mean stronger side effects like diarrhea, nausea, low platelets, or bleeding. Oddly, one study hinted quercetin might speed the enzyme up instead, so the direction isn't fully settled.

Honestly, this is mostly theoretical and based on weak, mostly lab-based evidence. Food amounts of quercetin almost certainly won't matter, but concentrated supplements might. Please check with your pharmacist or oncologist before combining them.

What Is Pacritinib Used For?

Brand name: Vonjo

Vonjo (pacritinib) is used to treat adults with intermediate or high-risk myelofibrosis (MF) — a serious bone marrow disease — including primary MF and secondary MF that developed after polycythemia vera or essential thrombocythemia (other blood disorders). It is specifically approved for patients whose platelet count is below 50 × 10⁹/L (very low platelets), a group that has limited treatment options.

This approval is based on demonstrated reductions in spleen size. Continued approval may depend on results from confirmatory trials showing broader clinical benefit.
