Why this applies to Pralsetinib

Pralsetinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your cancer medicine pralsetinib is processed by a liver enzyme called CYP3A4, and high-dose quercetin supplements may nudge that enzyme. Pralsetinib is active on its own (it isn't a prodrug), so if quercetin slows that enzyme down, more pralsetinib could linger in your blood. That might mean stronger side effects like high blood pressure, swelling, fatigue, low blood counts, or liver enzyme changes.

I'll be honest with you, this is mostly lab and theory level evidence, and quercetin's effect on this enzyme has even looked inconsistent in studies. Food amounts in things like onions and apples are very unlikely to matter. With a cancer drug like this, though, please run any concentrated quercetin supplement past your oncologist or pharmacist first.

Pralsetinib also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the situation with Gavreto (pralsetinib) and quercetin. Pralsetinib is carried in and out of your gut by a transporter called P-glycoprotein, a kind of pump that helps control how much drug actually gets absorbed. Quercetin, at concentrated supplement doses, may block that pump a bit. In theory, that could let more pralsetinib into your bloodstream, which might bump up side effects like high blood pressure, tiredness, swelling, constipation, mouth sores, or changes in your blood counts and liver labs.

I want to keep this in perspective. This is mostly theoretical and based on weak, mixed studies, and the everyday quercetin you get from foods is unlikely to matter. But since pralsetinib is a serious cancer medicine, please run any quercetin supplement past your oncologist or pharmacist first.

What Is Pralsetinib Used For?

Brand name: Gavreto

Gavreto (pralsetinib) is used to treat adults with metastatic RET fusion-positive non-small cell lung cancer (NSCLC) — a type of lung cancer that has spread and carries a specific genetic alteration detected by an FDA-approved test.

Gavreto is also approved for adults and children aged 12 and older with advanced or metastatic RET fusion-positive thyroid cancer who need systemic (whole-body) treatment and whose cancer no longer responds to radioactive iodine (when radioactive iodine is an appropriate option). This thyroid cancer use is under accelerated approval, meaning continued approval depends on results from confirmatory clinical trials.
