Relugolix, Estradiol, Norethindrone Acetate falls under P-GLYCOPROTEIN SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the connection worth knowing: Myfembree contains relugolix, which is carried in and out of your gut by a transporter called P-glycoprotein (P-gp). Quercetin, in concentrated supplement doses, may block that pump. If it does, a bit more relugolix could get absorbed than usual, which in theory could nudge up side effects like hot flashes, headaches, mood changes, or fatigue.

I want to be honest about how soft this evidence is. It's mostly theoretical and based on small studies with other drugs, not Myfembree itself, so I wouldn't lose sleep over it. The amount of quercetin in everyday foods is very unlikely to matter. Before starting a high-dose quercetin supplement, just run it by your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Relugolix, Estradiol, Norethindrone Acetate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Myfembree combines three active ingredients (relugolix, estradiol, and norethindrone), and parts of this combo are broken down by a liver enzyme called CYP3A4. Quercetin can nudge that enzyme in either direction. In some studies high-dose quercetin slows CYP3A4 (which could let these hormones build up a bit, leading to more nausea, headache, breast tenderness, or mood changes), while one study suggests it can actually speed the enzyme up (which could lower hormone levels slightly).

Here's the honest part: this is mostly lab and animal data, so it's theoretical and not well proven in people taking Myfembree. The amount of quercetin in everyday foods is very unlikely to matter. If you're thinking about high-dose supplements, just run it by your pharmacist or doctor first.

What Is Relugolix, Estradiol, and Norethindrone Used For?

Brand name: Myfembree

Myfembree is approved for two conditions in premenopausal women: managing heavy menstrual bleeding caused by uterine fibroids (non-cancerous growths in the uterus), and managing moderate to severe pain caused by endometriosis (a condition where tissue similar to the uterine lining grows outside the uterus).

Treatment with Myfembree should not exceed 24 months because of the risk of ongoing bone loss that may not fully reverse after stopping the medication.
