Here's the wrinkle with tamoxifen: it's actually a prodrug. Your body uses an enzyme called CYP2D6 to convert it into its powerful active form, endoxifen, which is what does the real work against breast cancer cells. So the usual rule flips here. In lab studies, quercetin appears to block CYP2D6, and blocking that enzyme could mean your body makes less endoxifen, potentially making tamoxifen less effective.

Now, the important part: this is only seen in test-tube research, not in actual people, so it's theoretical. Food amounts of quercetin (apples, onions, tea) are very unlikely to matter. Concentrated, high-dose supplements are the real question. Since tamoxifen is so important, please check with your oncologist or pharmacist before starting any quercetin supplement.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the wrinkle with tamoxifen: it's actually a prodrug. Your body has to convert it into its active form (called endoxifen) using liver enzymes before it can fight breast cancer. One of the enzymes involved in handling tamoxifen is CYP2C9, which quercetin appears to slow down in lab and small human studies.

So this isn't a simple "more drug equals more side effects" situation. Because tamoxifen needs to be activated, anything that interferes with those enzymes could in theory shift how much active medicine you make. The evidence here is mostly theoretical and based on other drugs, not tamoxifen directly, so I don't want to alarm you. Food amounts of quercetin are very unlikely to matter, but please run high-dose supplements by your oncologist or pharmacist first.

P-GLYCOPROTEIN SUBSTRATES

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist for tamoxifen and quercetin. Tamoxifen rides through your body partly on a transporter pump called P-glycoprotein. Quercetin, when taken in concentrated supplement doses, may slow that pump down. In theory, that could let a bit more tamoxifen get absorbed and raise its levels in your blood. If that happens, you might notice more of tamoxifen's usual effects, things like hot flashes, leg cramps, fatigue, or nausea.

Honestly, the evidence here is thin and mostly theoretical, based on small studies with other medicines, not tamoxifen itself. The amount of quercetin you'd get from foods like onions or apples is very unlikely to matter. Before adding a high-dose quercetin supplement, just run it by your oncologist or pharmacist first.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's a wrinkle worth knowing about tamoxifen. It's actually a prodrug, meaning your body has to convert it into its active form (mostly through liver enzymes) before it can do its job of blocking estrogen in breast tissue. Quercetin can nudge those same liver enzymes, but the lab and animal data we have point in different directions, some studies suggest it slows the enzymes while one human study hinted it might speed them up.

So honestly, we can't say with confidence which way tamoxifen would shift, and the evidence here is weak (mostly test-tube and animal work). Casual quercetin from foods like onions and apples is very unlikely to matter. Concentrated supplements are the question mark. Since tamoxifen is so important, please run any quercetin supplement past your oncologist or pharmacist first.

About Tamoxifen

What Is Tamoxifen Used For?

Brand names: Nolvadex, Soltamox

Tamoxifen is used to treat estrogen receptor-positive metastatic breast cancer in both women and men, and as follow-up (adjuvant) therapy after surgery for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.

In women with ductal carcinoma in situ (DCIS — an early, non-invasive breast change) following surgery and radiation, tamoxifen lowers the risk of developing invasive breast cancer. It is also approved to reduce the risk of breast cancer in women who are at high risk but have not yet been diagnosed. Both Tamoxifen Citrate tablets and Soltamox oral solution share these indications.
