Why Are There 2 Interactions?
Quercetin interacts with whole categories of medication. Tramadol, Acetaminophen falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Tramadol, Acetaminophen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.
In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the twist with Tramacet: the tramadol part is actually a prodrug. Your body uses an enzyme called CYP2D6 to turn it into the form that gives most of the pain relief. Quercetin may block that same enzyme, so in theory you could get a bit less of tramadol's active pain-relieving effect. The acetaminophen half works through a different pathway and isn't really affected here.
I want to be honest about how soft this evidence is. This comes only from lab (test tube) studies, and it has never actually been shown in real people. Normal food amounts of quercetin are very unlikely to matter. If you take high-dose supplements and notice your pain isn't as well controlled, just check in with your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Tramadol, Acetaminophen also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's something worth knowing about this combo. Tramadol is partly handled by an enzyme called CYP3A4, and quercetin can nudge that enzyme. The tricky part is that the lab and human data point in opposite directions: in some studies quercetin blocks CYP3A4 (which could let more tramadol build up), and in at least one study it actually sped up that enzyme. So we honestly can't say with confidence which way it would tip for you.
If tramadol levels rose, you might feel more drowsiness, dizziness, nausea, or constipation. If they dropped, your pain might be less controlled. Keep in mind the acetaminophen half isn't really affected here. This is mostly theoretical, and food amounts of quercetin are unlikely to matter. Before adding a high-dose supplement, just check with your pharmacist or doctor.
