Alogliptin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the short version on pairing resveratrol with your alogliptin (Nesina). The theory is that resveratrol can block a liver enzyme called CYP3A4, and since alogliptin is partly handled by that enzyme, blocking it could in theory let a little more of the drug build up. Alogliptin is an active medication (not a prodrug), so higher levels would mean a slightly stronger blood-sugar-lowering effect.

The good news is this concern is mostly theoretical, coming from lab studies. In real human research, even 3000 mg of resveratrol daily for 8 weeks didn't require any dose changes for CYP3A4 medications. Alogliptin also rarely causes low blood sugar on its own. Still, it's smart to mention any new supplement to your pharmacist or doctor before starting.
