Entrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the gist with Rozlytrek (entrectinib) and resveratrol. Your body relies on an enzyme called CYP3A4 to clear entrectinib out of your system. In lab dish studies, resveratrol can slow that same enzyme down. In theory, that could let entrectinib build up a bit, which might mean stronger side effects like dizziness, tiredness, swelling, constipation, or changes in heart rhythm.

The good news is that this is mostly a test-tube concern. In actual human research, even fairly high resveratrol doses didn't force any dose changes for CYP3A4 medicines. Since entrectinib is a serious cancer therapy, I'd still loop in your oncology team or pharmacist before adding resveratrol, just to be safe rather than guess.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC) — a type of lung cancer where the ROS1 gene is rearranged — as confirmed by an FDA-approved test.

Rozlytrek is also approved for adult and pediatric patients older than 1 month with solid tumors that have an NTRK gene fusion (a specific genetic alteration), detected by an FDA-approved test, that have spread or cannot be removed without causing serious harm, and have worsened after prior treatment or have no good treatment alternatives. This NTRK indication was granted accelerated approval based on tumor response rates; continued approval may depend on results from confirmatory trials.
