Resveratrol and Estradiol, Progesterone

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Resveratrol interacts with whole categories of medication. Estradiol, Progesterone falls into 2 of those categories, so it can interact with Resveratrol in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Estradiol, Progesterone

Estradiol, Progesterone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that resveratrol can inhibit CYP1A2 enzymes. However, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version for Bijuva (estradiol plus progesterone). Your body uses a liver enzyme called CYP1A2 to help clear estrogen. In test-tube studies, resveratrol can slow that enzyme down. In theory, a slower enzyme means a little more estrogen hanging around, which could nudge up side effects like breast tenderness, bloating, headaches, nausea, or spotting.

I want to be honest about how shaky this is. This comes only from lab research, not from real people, so it may not amount to much in everyday life. Casual amounts from grapes or berries almost certainly don't matter. Before adding a concentrated resveratrol supplement, just run it by your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Estradiol, Progesterone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Bijuva combines estradiol and progesterone, and both hormones are cleared by a liver enzyme called CYP3A4. In a test tube, resveratrol can slow that enzyme down a little. If that happened in your body, your hormone levels could drift up slightly, which in theory might mean more of the usual things people notice on hormone therapy, like breast tenderness, bloating, nausea, headaches, or spotting.

Here's the reassuring part: this is mostly lab-dish science. In actual human studies, even a hefty 3000 mg daily dose of resveratrol for two months did not require any change to CYP3A4 medications. So a real-world problem here is unlikely. Still, it's worth a quick mention to your pharmacist or doctor before adding resveratrol, and please don't change your Bijuva on your own.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

About Resveratrol

What is Resveratrol?

Resveratrol
  • Heart and blood vessel health
  • Antioxidant support
  • Anti-aging
  • Blood sugar and metabolic support
  • General anti-inflammatory support

Resveratrol is a plant compound found in red grapes, berries, and peanuts that is popular for heart health, anti-aging, and antioxidant support. While lab and animal studies are promising, strong human evidence for most uses is still limited. It appears generally well tolerated in supplement doses, but talk with your pharmacist or doctor before starting it.
