Mometasone, formoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that resveratrol can inhibit CYP2C19 enzymes. However, this interaction has not been reported in humans.

Here's the honest version: this flag comes from the idea that resveratrol can slow down a liver enzyme called CYP2C19, which might let drugs cleared by that enzyme build up a bit. But Zenhale (mometasone plus formoterol) is an inhaler, and it isn't mainly handled by CYP2C19. Mometasone is processed largely by a different enzyme (CYP3A4), and inhaled doses act mostly in your lungs with very little reaching the bloodstream.

So for this specific combination, I wouldn't lose sleep over it. The concern is theoretical and based only on lab studies, with nothing reported in real people. If you ever notice extra jitteriness, a fast or pounding heartbeat, or tremor, mention it. And before starting resveratrol, just run it by your pharmacist or doctor.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Your Zenhale inhaler combines two medicines, and both mometasone (a steroid) and formoterol (a long-acting bronchodilator) are partly cleared by a liver enzyme called CYP3A4. In lab dishes, resveratrol can slow this enzyme down a bit. The thinking is that if it slowed clearance in your body too, slightly more of these drugs could linger, which in theory might nudge up side effects like a fast or pounding heartbeat, jitteriness, or tremor from the formoterol.

Here's the reassuring part: this concern comes only from test-tube work, not real people. In fact, a human study using a hefty 3000 mg of resveratrol daily for 8 weeks showed no need to adjust CYP3A4 medicines. So for most folks this is very unlikely to cause trouble. Still, it's worth a quick mention to your pharmacist or doctor, especially if you notice new heart racing or shakiness.
