Here's the gist with perampanel (Fycompa) and resveratrol. Your body uses several liver enzymes to clear perampanel, and one of them is called CYP2C19. In lab dish studies, resveratrol can slow that enzyme down. If that happened in a person, perampanel could clear a bit more slowly and build up, which might mean more of its usual side effects like dizziness, drowsiness, trouble with balance or coordination, and irritability or mood changes.

The important thing to know: this is theoretical. It's only been seen in lab studies, never reported in real people, so I wouldn't lose sleep over it. Food amounts (grapes, berries) are very unlikely to matter, but before adding a concentrated resveratrol supplement, run it by your pharmacist or neurologist.

I want to be honest about the strength of this, though. This concern comes only from test-tube research, and it has never actually been reported in people taking these two together. So I'd call it more theoretical than proven. Still, before adding a resveratrol supplement, run it by your pharmacist or doctor so they can keep an eye on how you're feeling.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the gist for Fycompa (perampanel). Your body clears this seizure medicine using a liver enzyme called CYP3A4. In test-tube studies, resveratrol can slow that enzyme down a bit. If that happened in real life, perampanel could build up slightly and you might notice more of its usual effects, like dizziness, drowsiness, trouble with balance, or mood and behavior changes.

The reassuring part: this concern comes mainly from lab data, not from people. In an actual human study, even 3000 mg of resveratrol a day didn't require changes to CYP3A4 medicines. So a real-world problem is unlikely. Still, perampanel is potent, so check with your pharmacist or doctor before adding a resveratrol supplement, and watch for extra sleepiness if you do.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
