Tramadol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the deal with mixing resveratrol and tramadol. Your liver uses an enzyme called CYP3A4 to help break tramadol down. Resveratrol has been shown in test tubes to slow that enzyme a bit, which in theory could let a little more tramadol hang around in your body. That might mean stronger effects like drowsiness, dizziness, nausea, or constipation.

The good news: this is only lab-level evidence, and a real human study using a hefty 3000 mg of resveratrol daily didn't require any dose changes for CYP3A4 drugs. So for most folks this is a low-concern, mostly theoretical concern. Still, if you take tramadol regularly, it's worth a quick chat with your pharmacist or doctor before adding a resveratrol supplement.
