Tocilizumab falls under HEPATOTOXIC DRUGS — the group Rhubarb interacts with.

Theoretically, concomitant use of rhubarb with potentially hepatotoxic drugs might increase the risk of developing liver damage.
Some animal research suggests that anthraquinones in rhubarb might have hepatotoxic effects. Also, rhubarb use has been linked to at least 24 cases of liver injury, although details on the dose of rhubarb and duration of use in these cases is unclear.

Here's the concern with pairing rhubarb root with Actemra (tocilizumab). Tocilizumab can sometimes raise your liver enzymes or, less often, cause more serious liver irritation, which is why your doctor checks bloodwork while you're on it. Rhubarb root in concentrated supplement form contains compounds called anthraquinones, and there have been a handful of reports linking heavy rhubarb use to liver injury. The worry is simply that two things that can stress the liver might add up.

I want to be honest: this is a theoretical caution based on how rhubarb behaves, not strong proof. Eating rhubarb stalks in a pie is very different from taking concentrated root capsules. Still, if you're on Actemra, it's worth a quick chat with your pharmacist or doctor before starting any rhubarb supplement.

What Is Tocilizumab Injection Used For?

Brand name: Actemra

Tocilizumab injection is used to treat several serious inflammatory and immune-related conditions. Actemra (tocilizumab) is indicated for moderately to severely active rheumatoid arthritis in adults who haven't responded well to other disease-modifying drugs, giant cell arteritis in adults, systemic sclerosis-associated interstitial lung disease (SSc-ILD) to slow lung function decline, polyarticular and systemic juvenile idiopathic arthritis in patients 2 years and older, and severe or life-threatening cytokine release syndrome (CRS) caused by CAR T-cell therapy in adults and children 2 and older.

Actemra is also approved for hospitalized adults and children aged 2 and older with COVID-19 who are on corticosteroids and need supplemental oxygen or mechanical breathing support. Not every brand or route is approved for every use — your prescriber will choose the right form for your specific condition.
