Phentermine, Topiramate falls under CNS DEPRESSANTS — the group Saffron interacts with.
Clinical research shows that taking saffron extract 60 mg orally daily for 26 weeks can cause drowsiness and sedation. Animal research suggests that adding saffron to hexobarbital further increases sleeping and slows motor activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with Qsymia (phentermine plus topiramate) and saffron. The concern on file is fairly mild and mostly theoretical. Saffron, especially in concentrated supplement doses, can occasionally cause drowsiness. The topiramate part of Qsymia can already make some people feel tired, foggy, or a little slowed down, so the worry is simply that the two together might add up and leave you feeling more sleepy or mentally hazy than usual.
This isn't a dangerous chemical clash, and saffron used as a cooking spice is very unlikely to matter. Qsymia is not a prodrug here, so we're just talking about overlapping sedation, not a change in how the drug is broken down. If you take a saffron supplement, mention it to your pharmacist or doctor and watch how you feel, especially before driving.
