Saffron and Spironolactone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Spironolactone

Spironolactone falls under ANTIHYPERTENSIVE DRUGS — the group Saffron interacts with.

Theoretically, concomitant use of saffron with antihypertensive drugs might have additive effects.
Animal and human research suggests that saffron extract can decrease blood pressure.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: saffron, when taken as a concentrated supplement, may gently nudge your blood pressure downward on its own. Since spironolactone is already working to lower blood pressure and clear extra fluid, stacking the two could add up to a bigger drop than you expect. That might leave you feeling dizzy, lightheaded, or wobbly when you stand up too fast.

I want to keep this in perspective, though. This concern comes mainly from animal and lab data, so it's more of a theoretical heads-up than a proven problem. A pinch of saffron in your cooking is very unlikely to matter. If you're thinking about regular saffron capsules, just run it by your pharmacist or doctor first and keep an eye on your blood pressure.

From our pharmacist’s overview on the Spironolactone page

“Potassium is the supplement I check first for anyone on spironolactone (Aldactone, Carospir), because this water pill works by holding potassium in the body, and supplementing more on top of that can push blood levels too high. It is one of 206 supplement and herbal ingredients with documented interactions here, and the list warrants g…”

About Saffron

What is Saffron?

Saffron
  • Depression and low mood
  • Anxiety
  • PMS symptoms
  • Memory and cognition
  • Eye health (age-related macular degeneration)

Saffron is a costly spice that has shown promise in early studies for mild-to-moderate depression and some mood and PMS symptoms, but most research uses small trials, so results are not definitive. It is generally well tolerated in food and small supplement amounts, but high doses can be harmful and it should be avoided in pregnancy. Talk to your pharmacist or doctor before using saffron supplements.

About Spironolactone

What Is Spironolactone Used For?

Brand names: Aldactone, Spiroton, Carospir

Spironolactone tablets (including Aldactone) are used to treat moderate-to-severe heart failure, high blood pressure, and fluid retention (edema) caused by liver cirrhosis or nephrotic syndrome. They are also used to diagnose and manage primary hyperaldosteronism — a condition where the adrenal glands produce too much of the hormone aldosterone.

Spironolactone oral suspension shares several of these uses — heart failure, high blood pressure, and edema from liver cirrhosis — but is not the same as Aldactone and is not therapeutically interchangeable with it. Your prescriber will choose the right form for your situation.
