Norgestrel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the simple version. Your body uses an enzyme called CYP3A4 to help clear norgestrel, the progestin in your birth control pill. Lab studies (in test tubes only) suggest sage might slow that enzyme down a bit. If that happened in real life, norgestrel could build up slightly and you might notice more of its usual effects, like spotting, breast tenderness, headaches, or mood changes.

I want to be honest about how shaky this is. This has never actually been seen in people, and normal cooking amounts of sage are very unlikely to matter at all. Concentrated sage extracts or the essential oil are the bigger question mark. Before adding a strong sage supplement, just check with your pharmacist or doctor so your contraception stays reliable.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

What Is Progestin-Only (norethindrone) Oral Contraceptives Used For?

Brand names: Ovrette, Orval 21

Norethindrone progestin-only pills — sometimes called the "mini-pill" — are used solely to prevent pregnancy. They do not contain estrogen, which makes them an option for women who cannot or prefer not to use estrogen-containing contraceptives.

When taken perfectly (same time every day, no missed pills), fewer than 1 in 100 women will become pregnant in the first year. With typical use, that number rises to about 9 in 100 — mostly because of late or skipped pills. Consistency is everything with this method.
