Why this applies to Letrozole

Letrozole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the gist for Letrozole (Femara): your body clears it mainly through a liver enzyme called CYP3A4. Schisandra tends to slow that same enzyme down, so in theory letrozole could clear more slowly and build up a bit higher in your blood. Letrozole is not a prodrug, so more drug sitting around usually means a stronger effect and a higher chance of its usual side effects, things like hot flashes, joint and muscle aches, fatigue, headache, or feeling dizzy.

I want to be honest about the evidence though. This concern comes from lab and animal work, not from real patients taking these two together, so it's more of a caution than a proven problem. Since letrozole is treating something serious, please don't add or stop schisandra on your own. Run it past your oncologist or pharmacist first.

What Is Letrozole Used For?

Brand name: Femara

Letrozole (brand name Femara) is used to treat breast cancer in postmenopausal women whose tumors are driven by estrogen. It is used in three settings: as adjuvant (after-surgery) treatment for hormone receptor positive early breast cancer; as extended adjuvant treatment for women who have already completed about five years of tamoxifen therapy; and as first- or second-line treatment for locally advanced or metastatic (spread) breast cancer.

Femara is specifically indicated for first-line treatment of hormone receptor positive or receptor-unknown locally advanced or metastatic breast cancer, and for advanced breast cancer in women whose disease has progressed after antiestrogen therapy.
