Taking shilajit with antidiabetes drugs might increase the risk of hypoglycemia.
Most human and animal research shows that shilajit can decrease fasting plasma glucose levels. In an animal model, shilajit 100 mg per kg daily enhanced the glucose-lowering ability of both glibenclamide and metformin when given in combination over a 4 week period. Monitor blood glucose levels closely. Dose adjustments might be necessary.

Here's the simple version: Shilajit may have its own mild blood-sugar-lowering effect. So when you pair it with acarbose, the two could add up and push your glucose lower than acarbose alone would. Acarbose works differently from many diabetes pills (it slows how fast your meal turns into sugar), but stacking another glucose-lowering agent on top can still tip you toward a low.

I want to be honest about the evidence, though. This caution comes mostly from animal and lab studies, not solid human trials, so it's more of a 'keep an eye out' than a red alert. If you do try shilajit, watch for shakiness, sweating, dizziness, or feeling foggy, and check your glucose more often at first. Don't change your acarbose dose on your own. Loop in your pharmacist or doctor first, and pick a heavy-metal-tested product.
