Shilajit and Pramlintide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIDIABETES DRUGS

Why this applies to Pramlintide

Pramlintide falls under ANTIDIABETES DRUGS — the group Shilajit interacts with.

Taking shilajit with antidiabetes drugs might increase the risk of hypoglycemia.
Most human and animal research shows that shilajit can decrease fasting plasma glucose levels. In an animal model, shilajit 100 mg per kg daily enhanced the glucose-lowering ability of both glibenclamide and metformin when given in combination over a 4 week period. Monitor blood glucose levels closely. Dose adjustments might be necessary.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: Shilajit appears to lower blood sugar on its own. Pramlintide (Symlin) already works alongside your mealtime insulin to keep blood sugar from spiking, so layering shilajit on top could push your sugar too low. That raises your risk of hypoglycemia, which can show up as shakiness, sweating, a racing heart, confusion, or dizziness.

I'll be honest with you: this concern is based mostly on lab and animal studies, not strong human trials, so the real-world risk is uncertain. Still, since Symlin users are already prone to low blood sugar, it's worth caution. Don't stop or change anything on your own. Check in with your pharmacist or doctor first, and if you do use both, monitor your blood sugar more closely than usual.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)

By severity: 1 major · 208 moderate · 30 minor.

From our pharmacist’s overview on the Pramlintide page

“The single entry rated major on the pramlintide (Symlin) list is Siberian cocklebur, which has caused severe hypoglycemia in humans, not just in lab studies. That matters more than usual here because pramlintide is injected at mealtimes alongside insulin, and a dangerous low is already the main hazard of that pairing.”

About Shilajit

What is Shilajit?

  • Fatigue and low energy
  • Male fertility and testosterone support
  • General vitality and aging
  • Athletic performance
  • Cognitive support

Shilajit is a sticky, tar-like substance found in rocks of mountain ranges like the Himalayas, used in traditional Ayurvedic medicine for energy and vitality. Human evidence is limited and mostly small or early-stage, so its real benefits are uncertain. Quality and purity vary widely, and contamination with heavy metals is a real concern, so choose tested products and talk to your pharmacist before use.

About Pramlintide Injection

What Is Pramlintide Injection Used For?

Brand name: Symlin

SymlinPen (pramlintide) is used as an add-on treatment for adults with type 1 or type 2 diabetes who already use mealtime insulin but still haven't reached their target blood sugar levels — even with optimal insulin therapy.

It is not a replacement for insulin. It works alongside insulin, given right before major meals, to reduce the spike in blood sugar that happens after eating.
