St. John's wort induces CYP3A4 enzymes and increases metabolism of CYP3A4 substrates. Clinically significant interactions have been reported with St. John's wort products containing hyperforin 1 mg or more.

If you take a Sudafed Cold/Flu product (which combines dextromethorphan, the cough suppressant, with pseudoephedrine, the decongestant) along with St. John's wort, there is a different concern worth knowing about. St. John's wort speeds up certain liver enzymes, so it can lower the level of the cough-suppressing part faster than normal. That mostly means it may simply work less well for your cough, not that it becomes more dangerous from this enzyme effect alone.

The bigger worry is serotonin. Both St. John's wort and dextromethorphan can raise serotonin, and stacking them can rarely cause symptoms like agitation, fast heartbeat, sweating, or shakiness. Please check with your pharmacist or doctor before combining these, especially for everyday short-term cold relief.

Dextromethorphan, Pseudoephedrine also falls under SEROTONERGIC DRUGS.

Theoretically, St. John's wort might inhibit reuptake and increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Concomitant use of serotonergic drugs with St. John's wort can lead to increased adverse effects and increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstriction disorders.

Here's the concern with Sudafed Cold/Flu. The dextromethorphan part (the cough suppressant) gently nudges serotonin levels up in your brain. St. John's wort does the same thing. Stack them together and you can get too much serotonin activity, something we call serotonin syndrome. That can show up as agitation, a racing heart, shivering, sweating, muscle twitching, or feeling confused. The pseudoephedrine (the decongestant) adds its own stimulant push, so jitteriness, a faster heartbeat, and higher blood pressure are also possible.

To be fair, this warning is mostly theoretical, based on how these substances work rather than lots of real-world reports. A single dose may be fine for many people, but I'd still run it by your pharmacist or doctor first, especially if you take St. John's wort daily.
