Here's the concern with this combination. Drospirenone, which is often part of a birth control pill, is cleared from your body by a liver enzyme called CYP3A4. St. John's wort speeds that enzyme up, so it can flush the hormone out faster and leave less of it in your system. The practical worry is reduced birth control protection, meaning a higher chance of an unplanned pregnancy. You might also notice spotting or breakthrough bleeding as a clue that levels are dropping.

This is a well-recognized interaction, especially with products that contain real amounts of hyperforin. Please don't stop either one on your own. Talk with your pharmacist or doctor, since you may need a backup method or a different approach.

Drospirenone also falls under CONTRACEPTIVE DRUGS.

St. John's wort increases the clearance of contraceptive drugs and reduces their clinical effects.
Females taking St. John's wort and oral contraceptives concurrently should use an additional or alternative form of birth control. St. John's wort can decrease norethindrone and ethinyl estradiol levels by 13% to 15%, resulting in breakthrough bleeding, irregular menstrual bleeding, or unplanned pregnancy. Bleeding irregularities usually occur within a week of starting St. John's wort and regular cycles usually return when St. John's wort is discontinued. Unplanned pregnancy has occurred with concurrent use of oral contraceptives and St. John's wort extract. St. John's wort is thought to induce the cytochrome P450 1A2 (CYP1A2), 2C9 (CYP2C9), and 3A4 (CYP3A4) enzymes, which are responsible for metabolism of progestins and estrogens in contraceptives.

Here's the concern with pairing St. John's wort with your drospirenone birth control. This herb revs up liver enzymes (mainly CYP3A4) that break down the hormones in your pill. When those enzymes work faster, your body clears the hormone out quicker, so the levels protecting you drop. Studies have shown contraceptive hormone levels falling by roughly 13 to 15 percent.

In plain terms, that means weaker protection and a real risk of breakthrough or irregular bleeding, often within a week of starting the herb, plus the bigger worry of an unplanned pregnancy. This isn't theoretical; pregnancies have actually happened with this combo. Please don't stop either one on your own. Talk with your pharmacist or doctor about a backup or different birth control method first.
