St. John's wort increases the metabolism and reduces the levels of CYP3A4 substrates.
St. John's wort induces CYP3A4 enzymes and increases metabolism of CYP3A4 substrates. Clinically significant interactions have been reported with St. John's wort products containing hyperforin 1 mg or more.

Here's the gist for trazodone: your liver uses an enzyme called CYP3A4 to break it down. St. John's wort revs that enzyme up, so your body clears trazodone faster than usual. The result is lower trazodone levels in your blood, which can mean it just doesn't work as well for sleep or mood. Trazodone isn't a prodrug, so faster clearance simply weakens it.

On top of that, both trazodone and St. John's wort nudge serotonin upward, so combining them carries a small risk of too much serotonin (agitation, sweating, racing heart, shakiness). That's a real but less common concern. Please don't change anything on your own. Loop in your pharmacist or doctor first.

Theoretically, St. John's wort might inhibit reuptake and increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Concomitant use of serotonergic drugs with St. John's wort can lead to increased adverse effects and increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstriction disorders.

Here's the thing to know about pairing St. John's Wort with trazodone: both can raise serotonin levels in your body. Trazodone already nudges serotonin up as part of how it works for depression and sleep, and St. John's Wort may do the same. Stacking them could mean too much serotonin activity.

When serotonin runs high, you might notice agitation, shakiness, sweating, a fast heartbeat, or feeling restless. In rare cases this can build into serotonin syndrome, which is a real emergency. To be fair, this warning is mostly theoretical, based on how each works rather than lots of reported cases, so don't panic. Still, please check with your pharmacist or doctor before combining them rather than starting or stopping anything on your own.
