Ergotamine, Belladonna, Phenobarbital falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Let's keep this one in perspective. One ingredient in tangerine, called tangeretin, sped up the CYP3A4 enzyme in a lab dish. The ergotamine part of your combination pill is partly cleared by that same enzyme. So in theory, if tangerine nudged CYP3A4 to work faster, your body might break down ergotamine a little quicker, which could slightly soften its migraine-relieving effect.
Here's the reassuring part: when researchers actually gave people tangerine juice, it didn't meaningfully change how a similar drug was processed. So the real-world effect is considered unlikely and minor, and a tangerine with breakfast almost certainly won't matter. If you regularly use concentrated tangerine extracts or notice your medicine working differently, just mention it to your pharmacist or doctor.
