Sirolimus falls under IMMUNOSUPPRESSANTS — the group Thuja interacts with.

Thuja might stimulate immune function. Theoretically, taking thuja might decrease the effects of immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the concern with pairing these two. Sirolimus (Rapamune) works by quieting down your immune system so your body doesn't attack your transplanted kidney. Thuja, on the other hand, is thought to do the opposite, it may gently rev the immune system up. So in theory, taking thuja could work against your sirolimus and make it less effective at preventing rejection.

I want to be honest though: this is a theoretical concern based on how thuja behaves in lab studies, not real cases in transplant patients. But because protecting a transplant is so important, I'd play it safe. Please don't start thuja without checking with your transplant team or pharmacist first.

Thuja, also called northern white cedar or arborvitae, is a tree whose leaf and oil have a long history in folk and homeopathic medicine, especially for warts and immune support. Solid human evidence for these uses is very limited, and the undiluted oil contains thujone, which can be toxic. Talk with a pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.

What Is Sirolimus Used For?

Brand name: Rapamune

Sirolimus oral solution and tablets are used to help prevent organ rejection in kidney (renal) transplant patients aged 13 and older. They are also used to treat lymphangioleiomyomatosis (LAM) — a rare lung disease in which abnormal muscle-like cells invade lung tissue.

Fyarro (sirolimus, intravenous injection) is used to treat adults with locally advanced, unresectable, or metastatic malignant PEComa — a rare tumor that grows in soft tissue or organs. Hyftor (sirolimus topical gel) is used to treat facial angiofibroma (small benign skin growths on the face) associated with tuberous sclerosis in adults and children aged 6 and older.
