Durvalumab falls under IMMUNOSUPPRESSANTS — the group Thuja interacts with.

Thuja might stimulate immune function. Theoretically, taking thuja might decrease the effects of immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the wrinkle with this pairing. Durvalumab (Imfinzi) is a type of cancer immunotherapy called a checkpoint inhibitor. Instead of calming the immune system down like the classic immunosuppressants on this list, it actually does the opposite: it takes the brakes off your immune system so it can attack the tumor. Thuja is thought, mostly in lab studies, to stimulate immune activity too.

So the concern here is theoretical and not well studied in people. In short, layering an immune-stimulating supplement on top of a drug that's already revving up your immune system could, in theory, add to immune-related side effects like fatigue, rash, or inflammation in the lungs, colon, or other organs. We just don't have real data. Please don't start thuja while on Imfinzi without first running it by your oncology team.

Thuja, also called northern white cedar or arborvitae, is a tree whose leaf and oil have a long history in folk and homeopathic medicine, especially for warts and immune support. Solid human evidence for these uses is very limited, and the undiluted oil contains thujone, which can be toxic. Talk with a pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.

What Is Durvalumab Injection Used For?

Brand name: Imfinzi

Imfinzi is used to treat a wide range of cancers in adults. For lung cancer, it is approved for resectable non-small cell lung cancer (given before and after surgery), unresectable Stage III non-small cell lung cancer (after chemoradiation), metastatic non-small cell lung cancer, limited-stage small cell lung cancer (after chemoradiation), and extensive-stage small cell lung cancer.

Beyond lung cancer, Imfinzi is also approved for locally advanced or metastatic biliary tract cancer (cancers of the bile ducts and gallbladder), unresectable hepatocellular carcinoma (liver cancer), mismatch repair deficient (dMMR) advanced or recurrent endometrial cancer, BCG-naive high-risk non-muscle-invasive bladder cancer, muscle-invasive bladder cancer, and resectable gastric or gastroesophageal junction adenocarcinoma (stomach cancer at the junction with the esophagus). Most of these uses involve Imfinzi given alongside other chemotherapy drugs.
