Ertugliflozin, Metformin Hydrochloride falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs the other direction from most interactions. Your diabetes pill (Segluromet, which combines ertugliflozin and metformin) does not really change how the thyroid extract works through enzymes. Instead, this is about timing and absorption. Thyroid hormones absorb best on an empty stomach, and taking other oral medications or food too close together can bind up some of that hormone and lower how much your body picks up.

The practical fix is simple: take your thyroid extract by itself, ideally 30 to 60 minutes before you eat or swallow your Segluromet. This interaction is rated minor, but spacing the doses keeps both working well. Since over-the-counter thyroid extract is unstandardized, please talk with your pharmacist or doctor about whether you even need it.
