Here is the interaction we found between these two, with the evidence behind it.
Oral Drugs
Why this applies to Ruxolitinib
Ruxolitinib falls under Oral Drugs — the group Thyroid Extract interacts with.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here the concern flows in one direction: taking your ruxolitinib (Jakafi or Opzelura) at the same time as thyroid extract could blunt how well the thyroid hormone gets absorbed. Thyroid hormones are finicky about timing, and when other pills share the stomach with them, less hormone may make it into your system. Ruxolitinib itself is not weakened by this, and it is an active drug (not a prodrug), so it keeps working as usual.
The practical fix is spacing them out, since thyroid hormone absorbs best on an empty stomach. This is a minor, well-documented timing issue, not a dangerous one. Still, because over-the-counter thyroid extract is unpredictable, please check with your pharmacist or doctor before mixing the two.
What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.

From our pharmacist’s overview on the Ruxolitinib page
“Ruxolitinib (Jakafi for myelofibrosis and other blood cancers, Opzelura as a skin cream) is cleared by one liver enzyme, CYP3A4, and that single fact explains nearly all 141 supplements here, which is why this list deserves real caution. Three entries are rated major.”

About Thyroid Extract
What is Thyroid Extract?
- Hypothyroidism (underactive thyroid)
- Fatigue and low energy
- Weight management
- Sluggish metabolism
- General thyroid support
Thyroid extract supplements are made from dried animal thyroid gland and may contain real thyroid hormones (T3 and T4), but the amounts in over-the-counter products are not standardized and can be unpredictable. Self-treating a thyroid problem with these supplements can be risky, so always work with a doctor or pharmacist and use prescription thyroid medicine when treatment is needed.
About Ruxolitinib
What Is Ruxolitinib Used For?
Brand names: Jakafi, Opzelura
Ruxolitinib is used in two very different ways. Oral products (Jakafi and Jakafi XR) treat certain blood and bone marrow conditions and graft-versus-host disease. A topical cream (Opzelura) treats certain skin conditions.
Each use belongs to a specific product. Jakafi/Jakafi XR are not skin treatments, and Opzelura is not approved for the blood conditions.
