Why this applies to Tenecteplase

Tenecteplase falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Tocotrienols interacts with.

Concomitant use of tocotrienols with anticoagulant or antiplatelet agents might increase the risk of bleeding. However, this has not been reported in humans.
Taking tocotrienols orally inhibits experimentally-induced platelet aggregation in humans. Theoretically tocotrienols might increase the risk of bleeding if taken with antiplatelet or anticoagulant drugs. However, tocotrienols 400-800 mg daily have been used with aspirin and/or clopidogrel for 1 year with no clear cumulative antiplatelet effects and no reports of bleeding.

Here's the situation: tenecteplase (TNKase) is a powerful clot-busting drug given in the hospital, usually as a single quick injection during a heart attack or stroke. Its whole job is to break down clots fast, so bleeding is already the main thing the medical team watches for. Tocotrienols, a form of vitamin E, may slightly reduce how well your platelets stick together, so in theory stacking them could nudge bleeding risk up a little.

In real life, this is a very low concern. Tenecteplase is a one-time emergency treatment, not something you dose at home alongside a supplement, and the bleeding worry here is only theoretical. Studies haven't shown tocotrienols causing bleeding in people. Still, always tell your care team about every supplement you take so they have the full picture.
