The combination product you're asking about (Bellergal-S) contains phenobarbital, a barbiturate that already calms the brain and makes you sleepy. Valerian is thought to gently boost GABA, the brain's own settling-down signal. So stacking the two could pile sedation on top of sedation. In real life that might look like extra drowsiness, grogginess the next morning, dizziness, or feeling unsteady on your feet.

I'll be honest about the evidence here: this concern comes mainly from lab and animal studies, not strong human data, so it's more theoretical than proven. Still, since phenobarbital is a potent sedative, it's worth a quick conversation with your pharmacist or doctor before adding valerian, rather than guessing on your own.

Belladonna, Ergotamine, Phenobarbital also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP3A4.
Although some in vitro evidence suggests that valerian extract might inhibit or induce CYP3A4, clinical pharmacokinetic (PK) studies show that valerian does not have a clinically significant effect on the CYP3A4 enzyme. In one PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of midazolam, an accepted CYP3A4 probe-substrate. In another PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days modestly increases levels of alprazolam, a CYP3A4 substrate, suggesting mild inhibition of CYP3A4. However, this mild inhibition is unlikely to be clinically relevant.

Let's break this down. Bellergal-S combines three medicines, and the one that matters most here is phenobarbital, a barbiturate that calms the nervous system. On paper, valerian is processed by the same liver enzyme (CYP3A4) that handles part of this combo. But good clinical studies show valerian doesn't meaningfully change how fast that enzyme works, so a big swing in drug levels isn't expected.

The more practical concern is overlap in effect, not metabolism. Valerian is a mild sedative, and phenobarbital is already strongly sedating. Stacking them could leave you feeling extra drowsy, foggy, or unsteady. That's not a reason to panic, but it is worth a quick chat with your pharmacist or doctor before combining them, especially if you drive or take other calming medicines.
