Here's the gist of it: your body relies on a liver enzyme called CYP1A2 to clear caffeine out of your system. Lab studies suggest wheatgrass may speed up that enzyme, which would help your body break down caffeine a little faster. In plain terms, the caffeine portion of this medicine could wear off sooner or feel a touch weaker, so you might notice less of the alertness or pick-me-up you're expecting.
I want to be honest about how soft this evidence is. It comes only from test-tube and animal work, not from people, so it's theoretical at this point. For most folks, an occasional glass of wheatgrass juice is unlikely to make a big difference. If you take it daily and notice a change, just check in with your pharmacist or doctor.
