White pepper might increase levels of phenytoin. In humans, the piperine constituent of white pepper seems to increase absorption, slow elimination, and increase levels of phenytoin.

Here's the gist with this combo: piperine, the spicy compound in white pepper, may help your body absorb phenytoin a bit better and clear it more slowly. That can nudge phenytoin levels upward. Since phenytoin is a narrow therapeutic index drug, even small shifts can matter. If levels climb too high, you might notice things like dizziness, drowsiness, unsteady walking, slurred speech, or jerky eye movements.

Now, don't panic. This is largely a theoretical concern drawn from how piperine behaves, and the normal pinch of white pepper on your dinner is very unlikely to cause trouble. The bigger question is concentrated piperine or pepper supplements. Before adding any of those, check with your pharmacist or doctor, who can watch your phenytoin levels if needed.
