Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Zedoary interacts with.

Theoretically, zedoary might increase levels of drugs metabolized by CYP3A4.
In-vitro research shows that a methanol extract of zedoary strongly inhibits the CYP3A4 metabolism of the tyrosine kinase inhibitors lapatinib and sorafenib, and to a lesser extent, gefitinib.

Here's the basic idea with irinotecan (Onivyde) and zedoary. Your body uses a liver enzyme called CYP3A4 to help break down and clear irinotecan. In test-tube studies, zedoary extract appears to slow that same enzyme down. If that held true in people, more of the drug could linger in your system, which could mean stronger side effects like severe diarrhea, nausea, mouth sores, low blood counts, and a higher infection risk.

I want to be honest about the strength of this, though. It's theoretical, based only on lab research, not on actual patients taking the two together. Still, irinotecan is potent chemo with a narrow safety margin, so I'd play it safe. Please don't start zedoary without first running it by your oncology team or pharmacist.
