4Life Transform Woman contains 3 active ingredients. L-Citrulline is an amino acid that your body converts to another amino acid (L-arginine), which can relax blood vessels and improve blood flow.
Evening Primrose is a seed extract rich in a fatty acid called gamma-linolenic acid (GLA), traditionally used for hormonal balance. Velvet Bean (Mucuna pruriens) is a tropical seed extract that contains levodopa, an active compound similar to medications used in Parkinson's disease — it's included here for its dopamine-boosting properties.
The capsules also contain two inactive ingredients: vegetable capsule material and medium chain triglyceride oil.

The evidence supporting these ingredients is mixed and often limited. L-Citrulline is rated possibly effective for athletic performance, but possibly ineffective for muscle loss (sarcopenia); evidence is insufficient to rate it for heart health, heart failure, or diabetes.
Evening Primrose is rated likely ineffective for breast pain (mastalgia) and possibly ineffective for asthma; evidence doesn't yet support its use for acne, ADHD, or other conditions listed. Velvet Bean has insufficient evidence for anxiety, Parkinson's disease, rheumatoid arthritis, and other conditions in our data.

How safe is it?
Well-documented data
L-Citrulline and Evening Primrose are both generally well tolerated in short-term use, though long-term safety isn't fully studied. The most common side effects from L-Citrulline are gastrointestinal — heartburn and stomach discomfort.
Evening Primrose may cause abdominal pain, diarrhea, nausea, headache, or dizziness. Velvet Bean is also generally well tolerated, with reported diarrhea, flatulence, nausea, or headache; insomnia occurred in about 3% of people in one study.
Pregnancy data is mixed: L-Citrulline and Velvet Bean lack enough safety information and should be avoided during pregnancy; Evening Primrose is rated possibly safe. During breastfeeding, L-Citrulline safety is unknown (avoid it), Evening Primrose is rated possibly safe, and Velvet Bean should be avoided because its levodopa can lower milk supply.

The most serious concern is Velvet Bean (Mucuna pruriens), which contains levodopa — a compound that acts like a drug in your body. It has major-severity interactions with methyldopa (a blood pressure medication), MAOIs (a class of antidepressants), and levodopa itself (used for Parkinson's disease).
Taking Velvet Bean with any of these could trigger dangerous drops in blood pressure, a hypertensive crisis, or severe levodopa side effects.

Evening Primrose (Oenothera biennis) defatted seed extract
Anticoagulant/Antiplatelet Drugs
Theoretically, evening primrose oil may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Evening primrose oil contains gamma linolenic acid (GLA). There is preliminary clinical evidence that GLA can reduce platelet aggregation and prolong bleeding time.
Lithium
Theoretically, concomitant use of lithium with evening primrose oil might decrease lithium levels and effects.
In a case report, a patient on a stable dose of lithium for 10 years experienced a reduction in lithium levels after taking evening primrose oil 500 mg daily. Baseline levels were 0.69 mmol/L, which decreased to 0.37 mmol/L after 2 months and 0.23 mmol/L after 3 months of use. Lithium levels increased within 6 weeks of discontinuing evening primrose oil, to 0.73 mmol/L; no clinical effects were noted.
Lopinavir/Ritonavir (Kaletra)
Theoretically, evening primrose oil might increase the levels and effects of lopinavir.
In a case report, an HIV patient who took evening primrose oil (Efamol) along with lopinavir/ritonavir experienced an increase in serum levels of lopinavir to 15.2 mg/L. Six weeks after discontinuing evening primrose oil, levels of lopinavir returned to the normal range of 5-10 mg/L. When re-challenged with evening primrose oil for a week, the patient's lopinavir levels increased from 6.69 to 8.11 mg/L. It is suspected that evening primrose oil increases levels of lopinavir by inhibiting cytochrome P450 3A4 (CYP3A4), which metabolizes lopinavir. However, this effect has not been reported in other research.
Phenothiazines
Theoretically, taking evening primrose oil with phenothiazines might increase the risk of convulsions.
Evening primrose oil contains gamma-linolenic acid (GLA). There is some concern that taking supplements containing GLA might cause seizures, or lower the seizure threshold, when taken with phenothiazines. In one report, three patients with schizophrenia who had received phenothiazines developed EEG changes suggestive of temporal lobe epilepsy after starting treatment with GLA, although none experienced an actual seizure. In another report, two patients with schizophrenia who were stabilized on phenothiazines developed seizures when evening primrose oil 4 grams daily was added. One of these patients had a prior history of seizures. It is unclear whether evening primrose oil had any additive epileptogenic effects with the phenothiazines; there is no evidence that taking evening primrose oil alone causes seizures.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, evening primrose may increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that linoleic acid, a constituent of evening primrose oil, inhibits CYP2C9.
Velvet Bean (Mucuna pruriens) seed extract
Levodopa
Concomitant use can increase the risk of levodopa-related adverse effects.
Cowhage contains levodopa. Some cowhage products have been standardized to contain 75-400 mg of levodopa per dose.
Methyldopa (Aldomet)
Theoretically, concomitant use of cowhage and methyldopa might increase the risk of hypotension.
Cowhage contains levodopa. Use of levodopa with methyldopa might cause additive hypotension. In addition, methyldopa may inhibit peripheral decarboxylation of levodopa and increase levodopa levels in the central nervous system; avoid using.
Monoamine Oxidase Inhibitors (Maois)
Theoretically, concomitant use of cowhage and non-selective MAOIs might increase the risk of hypertensive crisis.
Cowhage contains levodopa. Use of levodopa with non-selective MAOIs might cause hypertensive crisis. However, this interaction has not been reported with MAO-B selective inhibitors such as selegiline.
Anesthesia
Theoretically, concomitant use of cowhage and anesthesia might increase the risk of arrhythmias.
Cowhage contains levodopa. Use of levodopa with cyclopropane or halogenated hydrocarbon anesthesia has led to arrhythmias. Other anesthetics have not been implicated. Use other anesthetics in patients taking cowhage or tell patients to stop taking cowhage at least 2 weeks before surgery.
Antidiabetes Drugs
Theoretically, concomitant use of cowhage and antidiabetes drugs might increase the risk of hypoglycemia.
Animal research shows that cowhage might have hypoglycemic effects.
Antipsychotic Drugs
Theoretically, use of cowhage might decrease the clinical effects of antipsychotic drugs.
Cowhage contains levodopa. Use of levodopa might counteract the antidopaminergic effects of antipsychotic medications.
Guanethidine (Ismelin)
Theoretically, concomitant use of cowhage and guanethidine might increase the risk of hypotension.
Cowhage contains levodopa. Use of levodopa with guanethidine might cause additive hypotension; avoid using.
Tricyclic Antidepressants (Tcas)
Theoretically, use of TCAs might reduce the levels and clinical effects of cowhage.
Cowhage contains levodopa. Use of TCAs might reduce the absorption of levodopa. Some case reports describe patients that developed hypertension and dyskinesia when taking both levodopa and TCAs.
L-Citrulline
Antihypertensive Drugs
Theoretically, concomitant use of L-citrulline with antihypertensive drugs might have additive effects and increase the chance of hypotension.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. However, a meta-analysis of 5 small clinical studies suggests that taking L-citrulline 3-6 grams daily for 1-8 weeks does not lower blood pressure when compared with control.
Phosphodiesterase-5 Inhibitors
Theoretically, concurrent use of phosphodiesterase-5 (PDE-5) inhibitors and L-citrulline might result in additive vasodilation.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. Theoretically, taking L-arginine with PDE-5 inhibitors might have additive vasodilatory and hypotensive effects. However, in studies evaluating the combined use of L-arginine and sildenafil for erectile dysfunction, hypotension was not reported.
