Lactobacillus acidophilus is generally well tolerated in healthy adults and children. The main caution is that live probiotic organisms can rarely cause serious infection — chiefly bacteremia (bacteria in the bloodstream) or sepsis — mainly in very ill or immune-compromised people.

Orally, some people report mild gastrointestinal side effects like bloating, flatulence, abdominal pain, or diarrhea, though these are uncommon. Bifidobacterium longum is also generally well tolerated but carries similar rare risks of infection in critically ill patients, particularly infants.
