Annatto is generally recognized as safe in food amounts — it's been used as a food coloring for decades. Concentrated supplement doses, however, are not well studied, so the safety data advises caution with higher doses unless your doctor approves.
A small number of people have reported allergic reactions to annatto, including rare severe reactions like anaphylaxis (a life-threatening allergic response), angioedema (throat/face swelling), and drops in blood pressure. Constipation has been reported rarely with annatto leaf.
One case report linked annatto food coloring to worsened irritable bowel syndrome symptoms; the patient improved after stopping annatto products. Pregnancy and lactation safety data is not on file for annatto — talk with your doctor or pharmacist for personalized guidance if you're pregnant, breastfeeding, or planning to be.
