Insulin determir falls under ANTIDIABETES DRUGS — the group Annatto interacts with.

Animal studies suggest that certain annatto extracts may have hypoglycemic or hyperglycemic effects. Theoretically, taking annatto with antidiabetes drugs could reduce the therapeutic effects of these drugs or increase the risk for additive hypoglycemia. Monitor blood glucose levels closely.

Here's the honest picture with Levemir (insulin detemir) and annatto. Animal research hints that annatto extracts might nudge blood sugar in either direction, either lowering it or raising it. If annatto lowers your sugar on its own, pairing it with your long-acting insulin could add up to low blood sugar (hypoglycemia), meaning shakiness, sweating, dizziness, or confusion. If it instead pushes sugar up, your insulin might seem to work less well, leaving readings higher than usual.

Keep in mind this is theoretical, based on lab and animal studies, not solid human data. The small amount of annatto used as a food coloring is very unlikely to matter. A concentrated supplement is the bigger question. If you take it, check your glucose more often and loop in your pharmacist or doctor before changing anything.

Levemir (insulin detemir) is used to improve blood sugar (glucose) control in adults and children with type 1 diabetes or type 2 diabetes. It is a basal insulin, meaning it works around the clock to keep background blood sugar levels steady.

Levemir is not recommended for treating diabetic ketoacidosis (a dangerous buildup of acids in the blood that requires a different type of insulin).
