Acetohexamide falls under ANTIDIABETES DRUGS — the group Annatto interacts with.

Animal studies suggest that certain annatto extracts may have hypoglycemic or hyperglycemic effects. Theoretically, taking annatto with antidiabetes drugs could reduce the therapeutic effects of these drugs or increase the risk for additive hypoglycemia. Monitor blood glucose levels closely.

Acetohexamide (Dymelor) is an older sulfonylurea that lowers your blood sugar by nudging your pancreas to release more insulin. The concern here is pretty mild and mostly on paper. In animal studies, annatto extracts pulled blood sugar in both directions, sometimes lower, sometimes higher. So in theory, pairing concentrated annatto supplements with this medicine could either add to its sugar-lowering punch (think shakiness, sweating, dizziness, or confusion from a low) or work against it and leave your numbers running high.

Honestly, the annatto used to color foods like cheese is very unlikely to cause any of this. The real question is concentrated supplement doses, and even then the human evidence is thin. If you do take an annatto supplement, just check your blood sugar a little more often and loop in your pharmacist or doctor before changing anything.

Annatto is a plant-based coloring made from the seeds of the achiote tree and is widely used as a safe food dye. While it has a long history in traditional medicine and contains antioxidant compounds, there is little high-quality human research supporting its use as a supplement.
