Ertugliflozin falls under ANTIDIABETES DRUGS — the group Annatto interacts with.

Animal studies suggest that certain annatto extracts may have hypoglycemic or hyperglycemic effects. Theoretically, taking annatto with antidiabetes drugs could reduce the therapeutic effects of these drugs or increase the risk for additive hypoglycemia. Monitor blood glucose levels closely.

Here's the honest picture: the concern with annatto and your Steglatro (ertugliflozin) is mostly theoretical, based on animal studies rather than solid human research. In those lab studies, annatto extracts pushed blood sugar in different directions, sometimes lower, sometimes higher. So in theory it could either add to ertugliflozin's sugar-lowering or work against it.

Ertugliflozin isn't a prodrug, so this isn't about your liver activating or breaking down the medicine. It's really about both things affecting blood sugar at the same time. If sugar dropped too far you might feel shaky, sweaty, dizzy, or foggy. The amount of annatto used to color food is very unlikely to cause trouble. Concentrated supplement doses are the wiser thing to ask your pharmacist or doctor about, and just keep an eye on your glucose readings.
