BiomeMD contains 18 ingredients, most of them live probiotic bacterial strains: multiple Lactobacillus species (including acidophilus, salivarius, plantarum, paracasei, casei, gasseri, rhamnosus, and reuteri), several Bifidobacterium strains (longum, bifidum, and animalis lactis), plus Bacillus coagulans and Streptococcus thermophilus. These are meant to support healthy gut bacteria.
The product also includes prebiotics—compounds that feed beneficial bacteria—in the form of fructooligosaccharides and xylo-oligosaccharides, as well as Tahl Gum. Inactive ingredients are cellulose, vegetable oil powder, and silica.
Does it work?
Moderate evidence
The evidence is mixed and depends on what you're trying to use it for. Lactobacillus acidophilus and its DDS-1 variant are possibly effective for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Bifidobacterium bifidum is possibly effective for IBS and respiratory tract infections. Bacillus coagulans is possibly effective for constipation and IBS.
For most other conditions listed—including anxiety, asthma, hay fever, cognitive decline, and athletic performance—the evidence is too limited to rate. Fructooligosaccharides, one of the prebiotic ingredients, also doesn't have enough evidence to support its use.
How safe is it?
Well-documented data
The probiotic strains in this product are generally well tolerated in healthy adults. That said, people with weakened immune systems or serious illness should talk to their doctor first—there have been rare cases of bacterial infection in critically ill patients using probiotics.
Common mild side effects when first taking the product can include bloating, flatulence, abdominal discomfort, and gas, especially when you start; a meta-analysis suggests these are less common than feared at typical doses. One small trial reported a couple of rashes and itching with a similar probiotic blend, though it's unclear if the probiotics were responsible.
Pregnancy and lactation safety data are limited—check with your doctor or pharmacist before using while pregnant or breastfeeding.

The biggest concern is that several of the live bacterial strains—including Lactobacillus acidophilus, Bifidobacterium longum, Bifidobacterium bifidum, Bacillus coagulans, Lactobacillus acidophilus DDS-1, and Lactobacillus gasseri—may have their effectiveness reduced if you take them at the same time as antibiotics. Antibiotics can kill the live organisms in the probiotic, so you'll want to space out when you take them.

Lactobacillus acidophilus DDS-1
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium longum
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
Bifidobacterium bifidum
Antibiotic Drugs
Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
