How safe is it?
Well-documented data
Butterbur is generally well tolerated, but side effects do occur. The most common are belching, diarrhea, drowsiness, fatigue, headache, itchy eyes, itching, rash, and stomach upset.
Rarely, it can cause liver damage (hepatotoxicity) because of the pyrrolizidine alkaloids it may contain—even some PA-free products have rare reports of liver injury. It can also rarely cause abdominal pain, nausea, and pain in your joints or limbs.
During pregnancy, butterbur is rated likely unsafe because there isn't enough safety data and there are possible liver and other risks. During breastfeeding, safety is unknown, so it's best to avoid it.
