Properly prepared elderberry extracts from ripe fruit are generally well tolerated. The main safety concern is raw or unripe berries, leaves, bark, and seeds, which contain compounds that can cause nausea, vomiting, diarrhea, dizziness, and other symptoms — but cooking eliminates this risk.
Since this is an extract from ripe fruit, that's not your primary worry. There isn't enough reliable safety data to recommend elderberry during pregnancy or while breastfeeding — so the safest approach is to avoid it in those situations unless your doctor says otherwise.
One rare case reported autoimmune hepatitis (liver inflammation) in a patient with an underlying autoimmune condition who took elderberry for years, but this is uncommon.

Formula
Elderberry fruit extract has been used for centuries as a to treat the cold and flu, both of which are caused by a virus. The syrup is believed to reduce the severity and duration of the infection if taken within 48 hours of the first symptoms.
