Here's the idea behind this pairing. Elderberry seems to nudge the immune system to be more active, bumping up signals like interleukins and tumor necrosis factor. Pomalidomide works differently, partly by calming or reshaping the immune response while it fights multiple myeloma. So in theory, the two could pull in opposite directions, and elderberry might blunt some of what pomalidomide is trying to do.

I want to be honest though: this is mostly theoretical, based on lab work and the way elderberry behaves, not on solid studies in people taking pomalidomide. It's a moderate, possible concern, not a sure thing. Since pomalidomide is a serious cancer medicine, please don't start or stop elderberry on your own. Just run it by your oncologist or pharmacist first so they can weigh in.

Elderberry is a popular herbal supplement, mainly taken to help with colds and flu. Some small studies suggest it may modestly shorten cold or flu symptoms, but the evidence is limited and not conclusive. Cooked berries and properly prepared products are generally well tolerated, but raw or unripe parts can cause nausea and should be avoided.

About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
