Here's the thing to keep in mind with elderberry and thalidomide. Thalidomide isn't a prodrug that needs activating, so this isn't about your body breaking the drug down faster or slower. Instead, it's about opposing actions on your immune system. Elderberry has been shown in lab work to nudge the immune system up, raising signals like interleukins and tumor necrosis factor (TNF). Part of how thalidomide works, especially for leprosy skin flares (ENL), is by calming TNF down.

So in theory, elderberry could push against what thalidomide is trying to do, possibly making it work a little less well. This is mostly theoretical and not strongly proven, so don't panic. Still, since thalidomide treats serious conditions, it's worth a quick chat with your doctor or pharmacist before adding an elderberry supplement.

Elderberry is a popular herbal supplement, mainly taken to help with colds and flu. Some small studies suggest it may modestly shorten cold or flu symptoms, but the evidence is limited and not conclusive. Cooked berries and properly prepared products are generally well tolerated, but raw or unripe parts can cause nausea and should be avoided.

About Thalidomide

What Is Thalidomide Used For?

Brand name: Thalomid

Thalomid (thalidomide capsules) is approved for two uses. First, it is used in combination with dexamethasone to treat newly diagnosed multiple myeloma — a cancer of plasma cells in the bone marrow.

Second, Thalomid is used on its own to treat the skin symptoms of moderate to severe erythema nodosum leprosum (ENL), a painful inflammatory reaction that can occur in people with leprosy. It can also be used as ongoing maintenance therapy to prevent ENL from coming back. Note: Thalomid is not approved as the sole treatment for ENL when moderate to severe nerve inflammation (neuritis) is also present.
