Thalidomide falls under IMMUNOSUPPRESSANTS — the group Andrographis interacts with.
Laboratory research suggests that andrographolide has immunostimulant activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with thalidomide: it works in part by calming or modulating your immune system, especially when used for the skin sores and inflammation of ENL in Hansen's disease. Andrographis appears to do the opposite in lab studies, where its andrographolide compounds seem to stimulate immune activity. So in theory, the herb could push against what thalidomide is trying to accomplish and make it less effective.
I'll be honest, this concern comes only from test tube and animal research, not from people, so it's far from proven. Still, since thalidomide is a serious medication for serious conditions, I'd rather you not gamble. Please check with your doctor or pharmacist before adding andrographis.

About Thalidomide
What Is Thalidomide Used For?
Brand name: Thalomid
Thalidomide is used for two conditions. The first is newly diagnosed multiple myeloma, a cancer of the bone marrow, where it is given together with dexamethasone.
The second is erythema nodosum leprosum (ENL), a painful inflammatory skin reaction linked to leprosy. It is used for short-term treatment of moderate to severe skin symptoms. It is also used long term to help prevent those skin symptoms from coming back. It is not meant to be used alone for ENL when moderate to severe nerve inflammation (neuritis) is present.
