Here is the interaction we found between these two, with the evidence behind it.
IMMUNOSUPPRESSANTS
Why this applies to Denosumab
Denosumab falls under IMMUNOSUPPRESSANTS — the group Andrographis interacts with.
Laboratory research suggests that andrographolide has immunostimulant activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here is the idea behind this one. Andrographis contains compounds (andrographolides) that, in lab and animal studies, seem to rev up the immune system. Denosumab (Prolia or XGEVA) works in part by quieting certain immune-related signals, so the concern is that a supplement nudging your immunity in the opposite direction could theoretically work against it.
I want to be straight with you: this is based only on test-tube and animal research, not real-world patient cases, so it is a theoretical worry, not a proven problem. Denosumab is not a prodrug, and andrographis is not known to change how it is processed. Still, since denosumab does affect your immune defenses (and raises infection risk on its own), it is worth a quick chat with your pharmacist or doctor before adding andrographis.

From our pharmacist’s overview on the Denosumab page
“The typical person on denosumab (Prolia, XGEVA) is protecting bone, from osteoporosis or from cancer that has spread there, and many also reach for echinacea, ashwagandha, or ginseng to prop up their immune system. That overlap is the whole list.”

What is Andrographis?
- Common cold and flu symptoms

- Fever
- Immune support
- Digestive complaints
Andrographis is a bitter Asian herb traditionally used for colds, flu, and infections, and some studies suggest it may ease cold symptoms and shorten how long they last.
