GLIPIZIDE (Glucotrol)

Theoretically, andrographis extract might increase the maximum concentration and area under the curve of glipizide; however, opposite effects are seen with the constituent, andrographolide. The clinical significance of this interaction is unclear.
Animal research suggests that andrographis extract taken orally with glipizide in diabetes-induced rats increases the maximum concentration and area under the curve of glipizide. However, the opposite effect is seen with the constituent, andrographolide, in which the maximum concentration and area under the curve are decreased when taken with glipizide.

Your combo pill contains glipizide, which lowers blood sugar by nudging your body to release more insulin. The piece of this combo that interacts with andrographis is the glipizide part. (Metformin works differently and isn't really the concern here.)

Here's the honest picture: this interaction is theoretical and comes only from animal and lab studies, not people. In rats, the whole andrographis extract slightly raised glipizide levels, while the purified andrographolide compound lowered them, so it pulls in opposite directions and the real-world effect is unclear. If glipizide levels did climb, you could in theory see low blood sugar (shakiness, sweating, dizziness, confusion). I wouldn't lose sleep over it, but if you want to try andrographis, check with your pharmacist or doctor first and keep an eye on your sugars.
